Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
If you are registering at Five Elms Medical Practice, you can also visit reception between 10am and 6pm to collect a paper registration form.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Once the form has been submitted the reception team will contact you within 2 working days to either confirm your registration or request further information.
