Important information – Upcoming changes related to your service provider at Victoria and Five Elms Medical Centre

As of 30 September 2025, East London NHS Foundation Trust (ELFT) will no longer hold the contract for Victoria and Five Elms Medical Centre. A temporary provider will be put in place to provide the same services as your current GP and we expect you to continue to see the same care teams, as staff working at the practice should transfer with the services. This means that as a registered patient you should not see any interruption in the services provided.

We will respond within 4 hours during opening times.

  • are registered at the surgery

If your problem doesn’t need to be seen urgently, you can request a routine appointment instead.

Before you start

Notice on GP Collective Action

You may have heard about GPs taking part in collective action, but please note that our practice is not part of this action at this time. We are open as usual, so please attend your GP appointments as planned. We will continue to provide prescriptions, manage long-term conditions, offer vaccinations, and make referrals, including urgent ones.

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
